Family Directory

The following are guidelines for drafting your own personal Family Directory. Consider providing the location of your Directory to a trusted family member or friend, so it may be easily accessed in the case of untoward circumstances. Some choose to record all of this information electronically, on a disc.

Family Members Personal Information:

  • Names
  • Addresses
  • Phone Numbers
  • E-mail Addresses
  • Social Security Numbers
  • Dates of Birth

Family Health Profiles:

  • Medications, purpose, and dosage
  • Pharmacy contact information: phone number and address
  • Describe any allergies
  • Describe any health conditions

Family Documents:

  • Wills
  • Trusts
  • Powers-of-Attorney
  • Stock Certificates
  • Statements for your Investments, Pension/Retirement, Insurance, and Bank Accounts
  • Insurance Contracts (Life, Disability, Long Term Care, and/or Health)
  • Savings Bonds
  • Mortgage Documents
  • Tax Returns
  • Pertinent Corporate Documents
  • Employee Benefits Information
  • Other important documents

Show your trusted advocate where you keep these documents. If they are in a Lock Box, make sure you provide them with the address and the location of the key.

Family Usernames and/or Passwords:

  • Bank Accounts
  • Bill Pay Accounts
  • Home Security Systems
  • Voicemails
  • Etc...

Technology has made life convenient; share the convenience with those who may step in to help!

Physicians, Specialists, Estate Planner, Accountant, Investment Advisor, Clergy/Rabbi:

  • Name
  • Address
  • Phone Numbers
  • Email Addresses

Emergency Contact(s)

  • Name
